Roast the Vegetables
Preheat your oven to 400°F (200°C). Toss the cherry tomatoes and unpeeled garlic cloves in olive oil, salt, and pepper. Spread on a baking sheet and roast for about 20-25 minutes until the tomatoes are blistered and the garlic is soft.

Cook the Pasta
While the vegetables are roasting, bring a large pot of salted water to a boil. Add the elbow macaroni and cook according to package instructions until al dente. Drain and set aside.

Make the Creamed Spinach
In a large skillet, heat a tablespoon of olive oil over medium heat. Add the spinach and cook until wilted. Add heavy cream, salt, black pepper, and red pepper flakes, stirring until combined. Cook until slightly thickened.

Combine Ingredients
Once the tomatoes and garlic are roasted, remove from the oven. Squeeze the garlic out of its skins and mash it into a paste. In a large bowl, combine the cooked macaroni, creamed spinach, roasted tomatoes, and garlic. Mix in half of the parmesan cheese.

Bake
Transfer the macaroni mixture to a baking dish. Sprinkle the remaining parmesan cheese and crispy onions on top. Bake in the oven at 350°F (175°C) for about 15-20 minutes until heated through and the top is golden.
