Prepare the Bread
Cut the stale bread into 1-inch cubes and place them in a large mixing bowl.

Add the diced apples to the bowl with the bread. If using, add the raisins as well. Toss gently to combine.

In a separate bowl, whisk together the milk, heavy cream, granulated sugar, brown sugar, eggs, vanilla extract, cinnamon, nutmeg, and salt until well combined.

Pour Mixture Over Bread
Pour the wet mixture over the bread and apple mixture. Make sure all the bread is soaked. Let it sit for about 30 minutes, allowing the bread to absorb the liquid.
