Reserve Pasta Water
Before draining the pasta, reserve about 1 cup of the pasta cooking water. Then, drain the pasta and set aside.
Sauce Preparation

Cook the Guanciale
In a large skillet over medium heat, add the guanciale. Cook until the fat is rendered and the guanciale is crispy, about 5-7 minutes.

Add Tomatoes
Once the guanciale is crispy, add the crushed tomatoes to the skillet. Stir to combine and let simmer for about 10 minutes, allowing the sauce to thicken slightly.

Combine Pasta and Sauce
Add the drained spaghetti to the skillet with the sauce. Toss to combine, ensuring the pasta is well coated. If the sauce is too thick, add reserved pasta water a little at a time until desired consistency is reached.

Remove from heat and add the grated Pecorino Romano cheese and freshly ground black pepper. Toss again until the cheese is melted and evenly distributed. Serve immediately, garnished with extra cheese and pepper if desired.
