Cook the Spaghetti
Bring a large pot of salted water to a boil. Add spaghetti and cook according to package instructions until al dente. Drain and set aside.

Prepare the Meatballs or Sausage
If using ground beef, mix it with minced garlic, chopped onion, oregano, salt, and pepper. Form into meatballs. For Italian sausage, remove the casing and crumble the sausage.

Cook the Meat
In a large skillet, heat olive oil over medium heat. Add meatballs or crumbled sausage and cook until browned and cooked through, about 8-10 minutes. Remove from skillet and set aside.

Make the Sauce
In the same skillet, add more olive oil if needed. Sauté chopped onions and minced garlic until fragrant. Add tomato sauce, salt, and pepper. Simmer for 10 minutes.

Combine
Add the cooked meatballs or sausage back into the sauce. Stir to combine and simmer for an additional 5 minutes.

Serve
Plate the spaghetti and top with the meat sauce. Garnish with grated Parmesan cheese and fresh basil if desired.
