Prepare the Cream Mixture
In a saucepan over medium heat, combine the heavy cream and vanilla bean (split and scraped) or vanilla extract. Heat until just boiling, then remove from heat and let it steep for 15 minutes.
Whisk Egg Yolks and Sugar
In a mixing bowl, whisk together the egg yolks and granulated sugar until the mixture is pale and slightly thickened.
Strain the Mixture
Strain the combined mixture through a fine-mesh sieve into a clean bowl to remove any cooked egg bits and the vanilla pod (if used).
Cool and Chill
Remove the ramekins from the water bath and let them cool to room temperature. Cover and refrigerate for at least 2 hours, preferably overnight.
Caramelize the Sugar
Before serving, sprinkle a thin layer of brown sugar over the top of each custard. Use a kitchen torch to caramelize the sugar until it forms a crispy layer. Alternatively, you can broil them in the oven for a few minutes, watching carefully to avoid burning.
Serve
