Cook the Guanciale
In a large skillet over medium heat, add the guanciale. Cook until it becomes crispy and golden, about 5-7 minutes. Remove excess fat if necessary.

Sauté the Vegetables
Add the chopped onion to the skillet and sauté for about 3-4 minutes until it becomes translucent. Then, add the minced garlic and chili flakes, cooking for an additional minute.

Add tomatoes
Stir in the crushed tomatoes and let the sauce simmer for about 10-15 minutes. Season with salt and juice from half the lemon.

Cook the Spaghetti
While the sauce simmers, cook the spaghetti in a large pot of salted boiling water until al dente, according to package instructions. Drain, reserving some pasta water.

Combine Spaghetti and Sauce
Add the cooked spaghetti to the skillet with the sauce. Toss to combine, adding reserved pasta water if needed to reach desired consistency.

Serve hot, garnished with grated Pecorino Romano cheese and a squeeze of lemon juice.
