In a large pot of salted boiling water, cook the spaghetti according to package instructions until al dente. Reserve 1 cup of pasta water, then drain the spaghetti.

Prepare the Guanciale
In a large skillet over medium heat, add the guanciale and cook until crispy, about 5-7 minutes. Remove excess fat if necessary, leaving about 1-2 tablespoons in the pan.

Make the Sauce
Add the red chili flakes to the skillet with the guanciale and cook for 30 seconds. Then, add the canned San Marzano tomatoes, breaking them up with a spoon. Simmer for about 10 minutes.

Combine Pasta and Sauce
Add the drained spaghetti to the skillet and toss to combine. If the sauce is too thick, add reserved pasta water a little at a time until desired consistency is reached.
