Clean the mushroom caps with a damp cloth and remove the stems carefully. Set the caps aside and chop the stems finely.

In a skillet over medium heat, add the Italian sausage and cook until browned and cooked through, breaking it up into small pieces as it cooks, about 5-7 minutes. Drain excess fat.

In a mixing bowl, combine the cooked sausage, softened cream cheese, grated Parmesan cheese, minced garlic, chopped parsley, and shredded mozzarella cheese. Mix until well combined. If using, add the four cheese blend.
