Make chocolate paste
In a small bowl or measuring cup, combine the unsweetened cocoa powder and granulated sugar. Add the hot water (about 1 tablespoon) and stir until you have a smooth, slightly thick chocolate paste with no lumps.

Combine with milk
Pour 1 cup of cold whole milk into a tall glass or mixing container. Add the smooth chocolate paste to the milk.

Flavor and season
Add 1 teaspoon vanilla extract and a small pinch (about 1/8 teaspoon) of salt to the milk and chocolate mixture. The salt brightens the chocolate flavor.

Serve
If serving cold, add ice (about 4 ice cubes) to a glass and pour the chocolate milk over the ice. Serve immediately. If you prefer it slightly warmed, gently heat the combined milk and chocolate paste in a small saucepan over low heat, stirring constantly, until warm (do not boil), then serve.
