Prepare the Elderberry Base
In a medium saucepan, combine the fresh elderberries and water. Bring to a boil over medium heat, then reduce to a simmer. Let simmer for about 30 minutes, until the elderberries have broken down and the liquid has reduced.

Strain the Mixture
Using a fine mesh strainer or cheesecloth, strain the elderberry mixture into a bowl, pressing down to extract as much liquid as possible. Discard the solids.

Combine Ingredients
In a blender, combine the strained elderberry juice, chopped apple, lemon juice, and cayenne pepper. Blend until smooth.

Serve
Pour the mixture into small shot glasses and serve immediately. Adjust cayenne pepper to taste.
