Prepare the Cream
In a saucepan, combine the heavy cream and split vanilla bean (or vanilla extract). Heat over medium heat until it just begins to simmer. Remove from heat and let it steep for 15 minutes.

Whisk Egg Yolks
In a mixing bowl, whisk together the egg yolks, granulated sugar, and a pinch of salt until the mixture is pale and slightly thickened.

Combine Mixtures
Slowly pour the warm cream into the egg yolk mixture, whisking constantly to prevent the eggs from scrambling. Remove the vanilla bean if used.

Prepare Ramekins
Preheat your oven to 325°F (160°C). Place ramekins in a baking dish and fill them with the custard mixture.

Bake
Pour hot water into the baking dish until it reaches halfway up the sides of the ramekins. Bake for 30-35 minutes, or until the edges are set but the centers are still slightly jiggly.

Cool and Chill
Remove the ramekins from the water bath and let them cool at room temperature. Then refrigerate for at least 2 hours, or until chilled.

Caramelize the Sugar
Sprinkle the granulated sugar evenly over the chilled custards. Using a kitchen torch, caramelize the sugar until it turns golden brown. Alternatively, you can place them under a broiler for a few minutes.
