Make the Base
In a medium saucepan, combine the whole milk, heavy cream, and sugar. Heat over medium heat until the sugar is dissolved and the mixture is hot but not boiling. In a separate bowl, whisk the egg yolks until smooth. Slowly pour the hot milk mixture into the egg yolks, whisking constantly to temper the eggs. Return the mixture to the saucepan and cook over low heat until thickened, about 5-7 minutes. Remove from heat and stir in the vanilla extract.

Cool the Base
Let the mixture cool to room temperature, then cover and refrigerate until chilled, at least 2 hours or overnight.
Churning

Churn the Gelato
Once chilled, pour the mixture into an ice cream maker and churn according to the manufacturer's instructions. For flavor variations, add cocoa powder, fruit puree, chocolate chips, or coffee during the last few minutes of churning.

Freeze the Gelato
Transfer the gelato to an airtight container and freeze for at least 4 hours to firm up before serving.
