Combine antibiotic powders

Weigh 1.0 g of amoxicillin trihydrate powder and 0.25 g of potassium clavulanate powder. Transfer both powders to a clean, dry mixing beaker. Gently blend the powders with a spatula until a uniform mixture is achieved to ensure even distribution of active components.

Pre-wet with glycerin

Add 5 ml glycerin to the powder blend. Stir gently to form a smooth paste. Glycerin helps disperse the powders and improve dissolution into the vehicle.

Prepare aqueous vehicle

Measure 50 ml of purified water into the mixing beaker (reserve 10 ml for final volume adjustments). Sprinkle 0.2 g xanthan gum slowly into the water while stirring vigorously to avoid clumping. Continue stirring until a uniform viscous vehicle forms (about 2–3 minutes).

Dissolve preservative and sweetener

Add 0.06 g sodium benzoate and 10 g sucrose to the viscous vehicle. Stir until the powders fully dissolve. Add 0.5 ml flavoring and stir to combine.

Incorporate antibiotic paste

Transfer the glycerin–antibiotic paste into the beaker containing the prepared vehicle. Rinse the mixing container with the reserved 10 ml purified water and add the rinse to the beaker to recover all active material. Stir thoroughly (1–2 minutes) to disperse the antibiotic uniformly in the vehicle.

Adjust pH

Add 0.03 g citric acid to the suspension and mix. Check pH with pH paper or meter; target pH for amoxicillin/clavulanate oral suspensions is typically around 4.5–5.5. If pH is too low, add small increments (0.1 ml) of dilute sodium hydroxide solution, stirring and rechecking pH after each addition until within target range. If pH is too high, add tiny amounts of citric acid and recheck.

Final volume and homogenization

Bring the total volume to approximately 60 ml with purified water (use the measuring cylinder). Stir the suspension vigorously for 1–2 minutes to ensure even distribution and a smooth texture. Allow the suspension to stand briefly (1–2 minutes) and re-stir to break any air bubbles.

Bottle and label

Transfer the finished suspension into an amber oral suspension bottle. Close the bottle tightly and label it with the simulated product name (Augmentin-like suspension), concentration (amoxicillin 1.0 g + clavulanate 0.25 g in ~60 ml), preparation date, storage instructions (store refrigerated 2–8°C), and an expiration (typically 7–14 days for compounded oral suspensions — for this simulation use 7 days).

Clean and calibrate: Ensure all measuring equipment and mixing vessels are clean and dry before use. If using a pH meter, calibrate per manufacturer instructions prior to pH adjustments.
