Combine dry ingredients
In a small bowl, whisk together the unsweetened cocoa powder and granulated sugar to remove lumps and ensure even distribution.

Warm the milk and cream
In a medium saucepan over medium-low heat, combine the whole milk and heavy cream. Warm gently until small bubbles form around the edge of the pan and steam rises, about 4–6 minutes. Do not boil.

Melt chocolate
Add the chopped semisweet chocolate to the saucepan. Continue to cook over low heat, stirring frequently, until the chocolate is fully melted and the mixture is glossy and uniform, about 2–3 minutes.

Keep warm
If not serving immediately, keep the hot chocolate warm over the lowest heat setting, stirring occasionally to prevent a skin from forming.
