Infuse milk & cream
In a medium saucepan, combine the whole milk and heavy cream. If using a vanilla bean, split lengthwise and scrape seeds into the milk, then add the pod. Warm the milk mixture over medium heat until it just begins to steam and small bubbles form at the edge (do not boil), about 5–7 minutes. Remove from heat and let steep 10 minutes to intensify vanilla flavor; if using extract, add it after removing from heat.
Prepare egg mixture
While the milk is steeping, whisk the egg yolks with 3 tablespoons granulated sugar and the salt in a medium bowl until the mixture is pale and slightly thickened, about 1–2 minutes.
Temper and cook
Remove the vanilla pod from the milk (if used). Slowly pour one ladle (about 1/4 cup) of the warm milk into the yolks while whisking constantly to temper. Continue adding milk in a slow steady stream until about half of the milk is incorporated. Return the tempered yolk mixture to the saucepan with the remaining milk, stir to combine, and cook over low heat, stirring constantly with a wooden spoon or spatula, until the custard thickens enough to coat the back of the spoon (170–175°F / 77–80°C), about 4–6 minutes. Do not let it boil.
Strain and cool
Pass the cooked custard through a fine-mesh sieve into a clean bowl to remove any curdled bits or vanilla pod remnants. Stir in vanilla extract now if you didn't use a bean. Cover the surface with plastic wrap to prevent a skin from forming and chill in the refrigerator until fully cold, at least 2 hours (or up to overnight).
Whip (optional lightening)
If you prefer a lighter texture (dolce rather than dense custard), whip the chilled custard briefly with a hand mixer for 30–45 seconds to incorporate a bit of air before serving.
Seasonal Fruit Compote
Prepare fruit
If using stone fruit, pit and dice into 1/2-inch pieces. If using pears, peel, core, and dice. If using berries, rinse and drain. Place fruit in a medium saucepan.
Cook compote
Add lemon juice, 2 tablespoons granulated sugar, and 2 tablespoons water to the fruit. Cook over medium heat, stirring occasionally, until fruit releases juices and thickens slightly, about 6–10 minutes for berries or 8–12 minutes for firmer fruits. For stone fruit or pears, stir in the fresh herb (e.g., 1 tablespoon torn thyme or basil) during the last 1–2 minutes of cooking. Taste and adjust sweetness — add up to 1 more tablespoon sugar if needed. Remove from heat and let cool to room temperature; compote will thicken as it cools.
Crunch / Garnish
Prepare crunch
Toast sliced or slivered almonds in a dry skillet over medium heat, stirring, until fragrant and lightly golden, 3–5 minutes; or roughly crush amaretti cookies. Allow to cool. Toss the cooled nuts or crushed cookies with 2 teaspoons demerara or turbinado sugar to add sparkle and crunch.
Assemble
Spoon a generous serving of chilled crème into individual dessert bowls or glasses (about 1/2 cup each). Top each with 2–3 tablespoons of the seasonal fruit compote. Sprinkle the sweetened crunch over the compote (about 1–2 tablespoons). Garnish with a small sprig of microherbs or mint leaf. If desired, add a light dusting of extra demerara sugar and quickly torch for a caramelized top (optional).
Serve immediately for best texture. The crème can be made 2 days ahead and the compote 24 hours ahead; store separately covered in the refrigerator and assemble just before serving.
