In a mixing bowl, combine the ground beef, salt, black pepper, garlic powder, and onion powder. Mix gently until just combined.

Shape the mixture into a burger patty that is approximately 12oz in weight and about ¾ inch thick.

Prepare the Bread
While the cheese is melting, toast the rye bread slices lightly in a toaster or on the grill.

On one slice of toasted rye bread, place the lettuce leaves, followed by the cooked burger patty with melted cheese and ham. Add slices of tomato on top, and finish with the second slice of rye bread.
