Break up chocolate
Meanwhile, break the Salvadoran chocolate disc into small pieces or roughly chop/grate dark chocolate so it will melt quickly and evenly.

Remove the cinnamon stick from the simmering liquid, then add the chocolate pieces and piloncillo (or brown sugar). Reduce heat to low and stir constantly with a wooden spatula or whisk until chocolate and sugar are fully dissolved and the mixture is smooth, about 3–5 minutes.

Stir in the ground cinnamon, a pinch of salt, and the vanilla extract. Continue simmering on low for 4–6 minutes, stirring occasionally, until the chocolate caliente is hot, well blended, and slightly thickened. Taste and adjust sweetness, adding up to 1 tablespoon more piloncillo or sugar if desired.

If you prefer a creamier, richer texture (common in Salvadoran homes), stir in the evaporated milk and heat through for another 1–2 minutes without boiling.
Serve

Pour the hot chocolate into warmed mugs. Top each serving with a dollop of whipped cream if using, and dust lightly with ground cinnamon. Sprinkle toasted sesame seeds or chopped almonds on top if desired for a traditional Salvadoran contrast.

Serve immediately while steaming hot, with sweet bread, tortillas, or Salvadoran quesadillas if available.
