Bloom cocoa
Place the unsweetened cocoa powder in a small bowl or measuring cup. Add the hot water (about 2 teaspoons) and stir vigorously until the cocoa forms a smooth paste with no lumps. This step releases flavor and eliminates graininess.

Sweeten and flavor paste
Add the granulated sugar, pinch of salt, and vanilla extract to the cocoa paste. Stir until the sugar is mostly dissolved and the mixture is smooth.

Combine with milk
Pour 2 cups of cold or slightly chilled whole milk into a large glass or small pitcher. Add the cocoa-sugar-vanilla mixture and whisk briskly (or use a milk frother) until fully incorporated and the drink is uniformly chocolate-colored. If using chocolate syrup for extra sweetness or visual swirl, add it now and whisk again.

Adjust sweetness and serve
Taste and adjust sweetness: add an additional 1/2–1 teaspoon sugar or a little more chocolate syrup if desired. Pour into serving glasses. Top with optional whipped cream or marshmallows if using. Serve immediately chilled.

Notes for hot chocolate variation
To make a hot version, heat the milk in a small saucepan over medium-low heat until steaming (do not boil). Whisk in the prepared cocoa paste and remove from heat once fully combined and hot. Serve warm.
