Prepare rim
Spread Old Bay seasoning and coarse sea salt on a small shallow plate and mix evenly. Rub the lemon wedge around the outer rim of a highball or pint glass to wet it, then dip the rim into the Old Bay mixture while rotating the glass to create an even rim. Set glass aside.
Cocktail
Combine ingredients
Fill a cocktail shaker halfway with ice. Add vodka, tomato juice, lemon juice, Worcestershire sauce, prepared horseradish, hot sauce, freshly ground black pepper, celery salt, pickle brine, and simple syrup (if using).
Shake and chill
Secure the shaker lid and shake vigorously for 10–12 seconds to chill, lightly dilute, and integrate the flavors. (Do not over-shake; a Bloody Mary is typically just shaken or rolled once.)
Garnish
Garnish the drink with a celery stalk and a lemon wedge on the rim. If desired, thread an olive, pickled okra, or cherry tomato on a cocktail pick and rest it across the glass. Serve immediately with a straw.
