Prepare Patties

Form and season

Divide the ground beef into two equal portions. Gently form each portion into a patty about 3/4"–1" thick and slightly larger than the buns (they will shrink while cooking). Make a shallow dimple in the center of each patty with your thumb to promote even cooking. Season both sides evenly with kosher salt, freshly ground black pepper, and a light drizzle of Worcestershire sauce (about 1/2 tsp per side).

Let the patties rest at room temperature for 10–15 minutes while you prepare other components—this helps them cook more evenly.

Bleu Cheese Topping

Make the bleu cheese spread

In a small bowl, combine the crumbled bleu cheese, 1 tablespoon softened unsalted butter, 2 tablespoons mayonnaise, minced chives, and 1/4 teaspoon freshly ground black pepper. Mash together with a fork until coarsely combined but still slightly textured. Taste and adjust seasoning. Cover and refrigerate until ready to use.

Caramelized Red Onions

Sauté onions

Heat 1 tablespoon unsalted butter in a skillet over medium heat. Add the thinly sliced red onion and a 1/2 teaspoon sugar. Cook, stirring occasionally, until the onions soften and begin to caramelize, about 8–10 minutes. Toward the end, add 1 teaspoon balsamic vinegar and cook 1 more minute to deglaze and concentrate flavor. Remove from heat and set aside.

Cook Patties

Pan-sear or grill

Heat a heavy skillet or grill over medium-high heat. Add 1 tablespoon vegetable oil to the skillet (if using a grill, oil the grates). When the pan is very hot, add the patties. Cook without moving until a brown crust forms, about 3–4 minutes. Flip and cook another 3–4 minutes for medium (adjust time for desired doneness). In the final 1 minute of cooking, reduce heat to medium-low and divide the bleu cheese spread between the two patties, allowing it to soften and slightly melt over the patties.

Remove patties to a plate and tent loosely with foil for 2–3 minutes to rest.

Toast Buns & Assemble

Toast buns

Lightly butter the cut sides of the buns or brush with a little oil and toast in a dry skillet or on the grill, cut-side down, until golden and crisp, about 1–2 minutes.

To assemble: place lettuce leaf on bottom bun (if using), set the rested patty with melted bleu cheese on top, add a spoonful of caramelized onions, tomato slices and pickles if desired, then top with the bun crown. Serve immediately.
