In a medium saucepan, combine the whole milk, heavy cream, sugar, and salt. Heat over medium heat until the sugar dissolves and the mixture is warm but not boiling.

Thicken the Base
Continue to cook the mixture over medium heat, stirring constantly until it thickens enough to coat the back of a spoon. This should take about 5–7 minutes.

Add Dulce de Leche
Remove the saucepan from the heat and stir in the dulce de leche and vanilla extract until fully incorporated.

Chill the Mixture
Transfer the mixture to a bowl and cover with plastic wrap, ensuring the wrap touches the surface to prevent a skin from forming. Chill in the refrigerator for at least 4 hours or overnight.

Churn the Gelato
Once the mixture is chilled, churn it in an ice cream maker according to the manufacturer’s instructions until it reaches a soft-serve consistency.
