Combine Ingredients
In a medium saucepan, combine the whole milk, heavy cream, and granulated sugar. Heat over medium heat until the sugar dissolves and the mixture is warm, but not boiling.

Cook the Mixture
Continue to cook the mixture over medium heat, stirring constantly, until it thickens enough to coat the back of a spoon, about 5-7 minutes. Do not let it boil.

Add Vanilla
Remove from heat and stir in the vanilla extract. Allow the mixture to cool to room temperature.

Prepare Flavor Base
For chocolate gelato, stir in the cocoa powder until fully dissolved. For strawberry gelato, fold in the pureed strawberries. For pistachio gelato, add the finely chopped pistachios. For coffee gelato, stir in the brewed coffee.

Chill the Mixture
Cover the mixture and refrigerate for at least 4 hours, or overnight, to allow the flavors to meld and the mixture to chill thoroughly.

Churn the Gelato
Pour the chilled mixture into an ice cream maker and churn according to the manufacturer's instructions until it reaches a soft-serve consistency.

Freeze the Gelato
Transfer the gelato to an airtight container and freeze for at least 4 hours to firm up before serving.
