Preheat the oven to 375°F (190°C). Line a baking sheet with parchment paper or lightly grease a shallow baking dish.

Wipe mushrooms clean with a damp paper towel. Remove and reserve stems by twisting gently; set caps aside on the baking sheet cap-side down.

Finely chop the reserved mushroom stems and set aside for the filling.

Brush mushroom caps lightly with 2 tsp olive oil and sprinkle with 1 tsp kosher salt and 1/2 tsp black pepper. This seasons them before stuffing and helps them roast.

Sausage Filling

Cook sausage and aromatics

Heat 1 tbsp olive oil in a skillet over medium heat. Add the ground sausage, breaking it up with a spoon. Cook until mostly browned, about 4–5 minutes.

Add the finely chopped onion and chopped mushroom stems to the skillet. Continue to cook until the onion is softened and the mushroom stems have released their moisture and browned slightly, about 4 minutes.

Stir in the minced garlic, 2 tbsp chopped parsley, 1/2 tsp dried oregano, and 1/4 tsp red pepper flakes. Cook 30–60 seconds until fragrant. Taste and adjust seasoning with a pinch more salt and pepper if needed. Transfer the mixture to a bowl and let it cool 5 minutes.

Binder & Cheese

To the warm sausage mixture, add 3/4 cup breadcrumbs, 1/2 cup grated Parmigiano-Reggiano, 2 oz softened cream cheese, 1 egg yolk, and 1/2 tsp lemon zest (if using).

Mix thoroughly until combined. The mixture should hold together when pressed; if too wet, stir in up to 1–2 tbsp more breadcrumbs. If too dry, add a teaspoon of water or a drizzle of olive oil.

Taste and correct seasoning (salt/pepper). Allow filling to cool until it's warm but not hot—this makes stuffing easier and prevents cracking the mushroom caps.

Assemble & Bake

Preheat oven (if turned off) to 375°F (190°C). Spoon or pipe about 1 to 1 1/2 tablespoons of filling into each mushroom cap, mounding slightly. You should have enough for 16 mushrooms; adjust amount per cap so all are evenly filled.

Sprinkle the tops with the remaining 2 tbsp grated Parmigiano-Reggiano.

Bake on the prepared baking sheet for 18–22 minutes, until the mushrooms are tender, the filling is heated through, and the cheese on top is melted and beginning to brown.

If you prefer a crispier top, place under a broiler for 1–2 minutes—watch closely to avoid burning.

Finish & Serve

Remove the stuffed mushrooms from the oven. Drizzle each with about 1 tsp olive oil if desired and sprinkle with 1 tbsp chopped fresh parsley for color and brightness.

Let rest 3–5 minutes before serving so filling firms slightly. Serve warm as an appetizer.
