Build the sauce
Reduce the heat to medium-low. Stir in the brown sugar and let it dissolve into the butter and onions for 1 minute. Add the ketchup, apple cider vinegar, Dijon mustard, Worcestershire sauce, hot sauce (if using), smoked paprika, black pepper, and water. Whisk or stir until smooth and combined.

Simmer the sauce
Bring the sauce to a gentle simmer, then cook for 4–6 minutes, stirring occasionally, until slightly thickened and glossy. Taste and adjust seasoning: add a pinch more salt or a teaspoon more vinegar or ketchup if you prefer tangier or sweeter sauce.

Add smokies and finish
Increase heat to medium-high and add the little smokies to the pan, stirring to fully coat them in the sauce. Bring to a gentle simmer, then reduce heat to low and continue to cook, stirring occasionally, for 8–10 minutes so the sausages heat through and absorb sauce flavors. If the sauce becomes too thick, add up to 2 tablespoons water to reach desired consistency.

Serve
Transfer the Little Smokies to a warmed serving bowl or an electric chafing dish to keep hot. Sprinkle chopped parsley over the top if desired. Serve with toothpicks for easy picking. Leftovers can be refrigerated in an airtight container for up to 3 days.
