Prepare aromatics
Peel and thinly slice shallots and garlic. Bruise and smash lemongrass stalks. Slice galangal. Tear kaffir lime leaves into rough pieces. Lightly crush Thai chilies to release aroma.

Make the broth base
In a pot, combine 4 cups of Tom Yum broth or water. Bring to a gentle simmer. Add lemongrass, galangal, kaffir lime leaves, and Thai chilies. Mix in Tom Yum paste if using. Simmer 5-7 minutes to infuse flavors.

Cook shrimp and season
Add large shrimp to the broth and cook 2-3 minutes until pink and just opaque. Stir in fish sauce, lime juice, and sugar. Adjust balance of salty, sour, and sweet.

Finish and serve
Taste and adjust the broth if needed. Remove lemongrass if desired. Stir in fresh cilantro. Ladle into bowls and serve with lime wedges on the side.
