    Dissolve sugar into malt

    In a small bowl, combine the granulated sugar and 2 tablespoons cold water. Stir until mostly dissolved. Add 3 tablespoons malted milk powder and stir until it forms a smooth, slightly thick paste (this helps prevent graininess).

    Combine liquids and flavor

    Pour 1 1/4 cups cold whole milk into a blender. Add 1 teaspoon vanilla extract.

    Add ice cream and malt paste

    Scoop 2 1/2 cups vanilla ice cream into the blender. Add the malt paste (from previous step) to the blender as well.

    Blend to desired consistency

    Secure the lid and blend on medium-high for 20–30 seconds until the mixture is smooth and thick but pourable. If mixture is too thick, add up to 2 tablespoons more milk and pulse again; if too thin, add a few tablespoons of ice cream and pulse. Taste and adjust sweetness or malt intensity by adding up to 1/2 tablespoon more malted milk powder (blend briefly).

Serve & Garnish

    Pour the shake into a chilled 16–20 oz glass, leaving room for garnish.

    Top with about 1/2 cup whipped cream if desired. Lightly dust the whipped cream with 1/2 teaspoon malted milk powder and sprinkle 2 tablespoons crushed malted milk balls for extra texture, if using.

    Finish with a maraschino cherry on top, if desired. Serve immediately with a straw and long spoon.
