Mix ingredients
In a medium bowl combine 1 lb lean ground beef, 1 large egg, 2 tbsp almond flour, 1 tsp low-sodium Worcestershire (or gluten-free tamari), 1 tsp garlic powder, 1 tsp onion powder, and 1 tsp black pepper. Gently mix with your hands or a fork until just combined — avoid overworking to keep patties tender.

Form patties
Divide mixture into four equal portions (about 4 oz / 113 g each). Shape each portion into a 3.5–4 inch wide, slightly thinner-in-the-center patty so it cooks evenly and stays flat. Press a shallow dimple in the center of each patty with your thumb.

Rest
Transfer cooked patties to a plate and rest 3–4 minutes to let juices redistribute.

Prepare toppings
While patties cook, wash and dry lettuce leaves. Slice tomato into 4 even slices, cut 4 red onion slices into rings if not already, and slice half an avocado thinly. If using portobello bun alternative, wipe caps clean and remove stems; brush each cap lightly with 1/2 tsp olive oil and season with a pinch of pepper, then grill 2–3 minutes per side until tender.

Assemble burgers
On a lettuce leaf (or grilled portobello cap if using), place a cooked patty. Top with a tomato slice, red onion ring(s), 1/2 avocado slices, 1 slice of reduced-fat cheddar if used, a drizzle (about 1 tsp) of sugar-free mustard and 1 tsp sugar-free ketchup or low-sodium Worcestershire if desired, and 1 sliced pickle. Finish with another lettuce leaf (or second portobello cap) to serve as a bun.

Serve
Serve immediately while patties are warm. Optionally pair with a simple side salad of mixed greens and a squeeze of lemon for extra freshness while keeping carbs and sodium low.
