Make chocolate syrup
Place the unsweetened cocoa powder and granulated sugar in a small bowl or cup. Add the hot water (about 2 tablespoons) and stir vigorously until you form a smooth, lump-free chocolate syrup. The hot water helps dissolve the cocoa and sugar; if small lumps remain, press them against the side of the bowl with the back of a spoon until smooth.

Combine with milk
Pour the 2 cups of cold whole milk into a tall glass or a pitcher. Add the prepared chocolate syrup to the milk.

Flavor and adjust
Add the 1 teaspoon vanilla extract and a pinch of salt to the milk and chocolate mixture. Stir or whisk briskly for 20–30 seconds until the chocolate is evenly dispersed and the milk has a uniform color. Taste and adjust sweetness by adding a little more sugar (½ teaspoon at a time) if desired.

Serve
Pour into serving glasses if mixed in a pitcher. If desired, top with whipped cream or marshmallows. Serve cold with ice, or refrigerate for 10–15 minutes to chill further. Stir briefly before serving if the cocoa has settled.
