Prepare the Onions
Slice the medium onion into thin rings. In a skillet over medium heat, melt 1 tablespoon of butter and add the onions. Cook, stirring occasionally, until the onions are caramelized and golden brown, about 10-15 minutes.

Cook the Beef
In a separate bowl, season the ground beef with salt and black pepper. Form into two patties. In the same skillet used for the onions, add another tablespoon of butter and increase the heat to medium-high. Cook the beef patties for about 3-4 minutes on each side or until they reach the desired doneness.

Assemble the Patty Melt
Spread mayonnaise on one side of each slice of buttermilk bread. On the unbuttered side of two slices, layer the cooked beef patties, caramelized onions, Swiss cheese, and pickles. Top with the remaining slices of bread, mayo side facing in.

Serve
Remove from the skillet, slice in half, and serve hot with extra pickles on the side if desired.
