Bring a large pot of salted water to a boil. Add the spaghetti and cook according to package instructions until al dente. Reserve about 1 cup of pasta water, then drain the rest.

Sauté the Guanciale
In a large skillet over medium heat, add the guanciale. Cook until crispy, about 5-7 minutes. Remove excess fat if necessary, leaving about 1 tablespoon in the pan.

Add the minced garlic and crushed red pepper flakes to the skillet. Sauté for an additional minute until fragrant.

Prepare the Tomato Sauce
Add the canned San Marzano tomatoes to the skillet, breaking them up with a spoon. Season with black pepper. Let the sauce simmer for about 10 minutes to thicken.
Combine and Serve

Combine Pasta and Sauce
Add the drained spaghetti to the skillet with the sauce. Toss to combine, adding reserved pasta water a little at a time to reach the desired consistency.

Remove from heat and stir in grated Pecorino Romano cheese. Adjust seasoning with more black pepper if desired.
