Cheesecake Shake Base
Soften and combine cheeses
Place the softened cream cheese and mascarpone in the bowl of a blender (or a large food processor). Beat briefly on low or pulse to smooth the cheeses so there are no lumps (about 20–30 seconds).
Add dairy and sweetener
Add the whole milk, heavy cream, vanilla ice cream, and granulated sugar to the blender with the cheeses.
Blend until creamy
Secure the lid and blend on medium-high until the mixture is very smooth and creamy, about 45–60 seconds. Scrape down the sides once and pulse again if needed. The texture should be thick but drinkable; adjust with up to 2 tablespoons of extra milk if too thick.

Incorporate espresso and liqueur
Add the cooled brewed espresso, coffee liqueur, amaretto (if using), instant espresso powder (if using), and unsweetened cocoa powder to the blender. Blend on low for 15–20 seconds to incorporate and to create the tiramisu flavor. Taste and adjust: add up to 1 tbsp more sugar if you prefer it sweeter or another 1 tbsp coffee liqueur for stronger coffee/boozy flavor.
If mixture becomes too thin after adding espresso, add 1/4 cup more ice cream or 2–3 ice cubes and pulse until desired thickness is reached.
Quickly dip each ladyfinger into the cooled cup of coffee for 1–2 seconds per side—do not soak until soggy. Reserve two whole dipped ladyfingers for garnish (optional); break the rest into large pieces.
For a layered drink: place a few broken coffee-dipped ladyfinger pieces into the bottom of each serving glass. For a blended inclusion: add 3–4 broken soaked pieces to the blender and pulse 2–3 times to create cookie flecks throughout the shake. If you prefer the cookies entirely separate, skip adding them to the blender and keep them for garnish.
Pour the shake into chilled tall glasses over the cookie layer if using. Top with a generous dollop of whipped cream (optional). Dust the whipped cream with the reserved cocoa powder and sprinkle chocolate shavings or curls over the top. Garnish with the reserved whole soaked ladyfingers or a broken piece on the rim.
For a thicker, frozen milkshake version: transfer blended shake to freezer for 15–20 minutes, then stir and serve. Let stand a minute before drinking so it softens slightly.
