Boil pasta
Bring 4 cups of water to a rolling boil in a large pot. Add 1 tablespoon salt. Add the 12 oz dried fettuccine and cook uncovered according to package instructions until al dente (usually 8–11 minutes). Stir occasionally to prevent sticking.
When pasta is al dente, reserve 1/2 cup of the cooking water, then drain the pasta well and set aside. Do not rinse.
Alfredo sauce
Start the roux and garlic
In a large skillet or sauté pan over medium heat, melt 6 tablespoons unsalted butter. Add the minced garlic and cook 30–45 seconds, stirring, until fragrant but not browned.
Make roux
Sprinkle 1 tablespoon flour over the melted butter and garlic. Stir constantly with a whisk or spoon for 1 minute to cook the flour and form a light roux.
Add dairy
Slowly whisk in 1 cup whole milk and 1 cup heavy cream, combining completely and smoothing any lumps. Increase heat to medium-high and bring the mixture to a gentle simmer, stirring frequently, until it slightly thickens (about 3–5 minutes).
Finish sauce
Lower heat to medium-low. Stir in 1 cup freshly grated Parmesan cheese until melted and smooth. Season with 1 teaspoon salt, 1 teaspoon freshly ground black pepper, and a pinch of freshly grated nutmeg (if using). Taste and adjust seasoning.
Adjust consistency
If the sauce is too thick, whisk in up to 1/2 cup reserved pasta cooking water, a tablespoon at a time, until you reach a silky sauce that will coat the fettuccine.
Keep sauce warm on the lowest heat while assembling; do not boil aggressively once cheese has been added to avoid graininess.
Assemble & bake
Transfer to baking dish
Preheat broiler to high. Transfer the coated fettuccine to a shallow 9x13-inch or similar ovenproof baking dish. Spread evenly.
Top with cheeses
Evenly sprinkle 1 cup shredded mozzarella over the pasta, then sprinkle the 1/4 cup extra grated Parmesan on top.
Melt cheese
Place the baking dish under the preheated broiler 6–8 inches from the heat source. Broil 2–4 minutes, watching closely, until the mozzarella is melted and beginning to bubble and lightly brown. Remove as soon as desired browning is achieved to avoid burning.
Let rest 2 minutes, garnish with 1 tablespoon chopped fresh parsley if using, then serve hot.
