Preheat and pan
Preheat oven to 350°F (175°C). Grease a 9x5-inch loaf pan with butter or nonstick spray and line with parchment paper leaving a 1-inch overhang for easy removal.
If using frozen cranberries, leave them frozen until ready to fold into batter to help them hold their shape and prevent bleeding color into the batter.
Cranberry prep
In a small bowl, toss the fresh or frozen cranberries with 2 tablespoons granulated sugar and 1 teaspoon flour. The sugar sweetens and the flour helps prevent them from sinking in the batter.
If using orange zest, stir it into the cranberries now so the flavor infuses while you prepare the batter.
Make the cream cheese swirl
In a medium bowl, beat the softened cream cheese with a whisk or electric mixer until smooth and free of lumps (about 1–2 minutes).
Add 1/2 cup powdered sugar, 1 egg yolk, and 1/2 teaspoon vanilla extract. Beat until fully combined and smooth. Scrape down the bowl and set the cream cheese mixture aside.
Make the batter
In a large bowl, whisk together 3 cups all-purpose flour, 1 cup granulated sugar, 2 teaspoons baking powder, and 1/2 teaspoon salt until evenly combined.
In a separate medium bowl, whisk the 2 large eggs until combined. Add 1 cup whole milk, 1/2 cup melted and cooled unsalted butter, and 1 tablespoon vanilla extract; whisk until smooth.
Pour the wet ingredients into the dry ingredients and stir gently with a spatula or wooden spoon until just combined. Do not overmix; a few small lumps are fine.
Fold the sugared cranberries into the batter gently so they are evenly distributed. If you want a lighter crumb, reserve 1/4 cup of cranberries to press on top before baking.
Assemble loaf
Spoon about half of the batter into the prepared loaf pan and spread it into an even layer.
Drop spoonfuls of the cream cheese mixture over the batter in an even line down the center. Use the back of a spoon to gently smooth but not blend it into the batter.
Spoon the remaining batter over the cream cheese layer, covering it. Smooth the top gently with an offset spatula. If desired, scatter the reserved cranberries and sprinkle the turbinado sugar and sliced almonds over the top.
Use a knife or skewer to make 2–3 gentle swirls through the top to create a marbled effect between batter and cream cheese—do not over-swirl.
Bake and cool
Bake in the preheated 350°F (175°C) oven for 55–70 minutes. Start checking at 50 minutes: tent loosely with foil if the top is browning too quickly. The loaf is done when a toothpick inserted into the center (not the cream cheese swirl) comes out with a few moist crumbs but no wet batter, and the internal temperature reads about 200°F (93°C).
Remove the pan from the oven and set on a wire rack. Let the loaf cool in the pan for 10–15 minutes to set, then lift it out using the parchment overhang and transfer to the rack to cool completely (about 1–1.5 hours) before slicing. Cooling prevents the cream cheese from oozing out when sliced.
Serve
Slice with a sharp serrated knife, wiping the blade between cuts if the cream cheese sticks. Serve at room temperature. Keeps wrapped in the refrigerator for up to 4 days; bring to room temperature before serving for best texture.
