Prepare cold glass
If desired, chill a coupe or rocks glass in the freezer for 5–10 minutes while you prepare the cocktail.
Dry shake (if using egg white)
Add blanco tequila, fresh lime juice, fresh grapefruit juice, St. Germain, agave syrup, ghost pepper tincture (start with 1–2 drops), and the egg white into a cocktail shaker (no ice). Seal and shake vigorously for 15–20 seconds to emulsify and build froth.
Wet shake
Open the shaker and add a large handful of ice (about 1 cup). Seal and shake vigorously for 12–15 seconds until the shaker is very cold to the touch and well diluted.
Strain and serve
Double-strain the cocktail into the chilled coupe or a rocks glass over fresh ice (or neat if you prefer). Use a fine mesh strainer to remove any ice shards and egg white solids for a smooth texture.
Adjust heat
Taste carefully. Add an additional drop of ghost pepper tincture if you want more heat — mix gently with a bar spoon. Remember ghost pepper is intensely hot; add sparingly.
Garnish
Express a grapefruit twist over the drink to release oils, then rub the twist around the rim and drop it in the glass.
Lightly sprinkle a small pinch (about 1/4 tsp) of dried chili flake or smoked sea salt on top of the foam or on the surface of the drink for aroma and a hint of savory heat.
Serve immediately and advise guests that the cocktail has a progressive heat — it will build on the palate.
