Use the following exact wording (or your local jurisdiction's required language): CONSUMING RAW OR UNDERCOOKED MEATS, POULTRY, SEAFOOD, SHELLFISH OR EGGS MAY INCREASE RISK OF FOOD-BORNE ILLNESS ESPECIALLY IF YOU HAVE CERTAIN MEDICAL CONDITIONS. Use all-caps for the heading if desired for emphasis.
Design layout
Place the advisory in a prominent location on menus, menu boards, or menu inserts near items that contain or may be served undercooked. If using an icon (recommended), place it immediately before the advisory or next to affected menu items.
Placement per item (if required)
