Prepare the Custard

In a saucepan, combine the milk, heavy cream, sugar, cocoa powder, and a pinch of salt. Heat over medium heat until hot but not boiling. In a bowl, whisk the egg yolks. Gradually add the hot mixture to the yolks while whisking continuously. Return the mixture to the saucepan and cook on low heat, stirring constantly until thickened. Remove from heat, stir in vanilla extract, and let cool.

Make the Ice Cream Base

In a saucepan, combine the heavy cream, milk, sugar, orange zest, and orange juice. Heat until sugar dissolves. In a bowl, whisk the egg yolks. Gradually add the hot mixture to the yolks while whisking. Return to the saucepan and cook on low heat until thickened. Cool completely in the refrigerator, then churn in an ice cream maker according to the manufacturer's instructions.

Prepare the Crumble

Preheat the oven to 350°F (175°C). In a bowl, mix flour, cocoa powder, sugar, and cocoa nibs. Cut in melted butter until crumbly. Spread on a baking sheet and bake for about 15 minutes or until crispy. Let cool.
