Prepare the Base
In a medium saucepan, combine the whole milk and heavy cream. Heat over medium heat until the mixture is warm but not boiling.

Whisk Together Sugar and Egg Yolks
In a separate bowl, whisk together the granulated sugar and egg yolks until the mixture is pale and creamy.

Cook the Custard
Return the mixture to the saucepan and cook over medium heat, stirring constantly until it thickens enough to coat the back of a spoon.

Add Vanilla
Remove from heat and stir in the vanilla extract. Allow the custard to cool to room temperature.

Once the custard is cool, mix in the chocolate chips and cocoa powder until well incorporated.

Cover the mixture with plastic wrap and refrigerate for at least 4 hours, or overnight for best results.

Pour the chilled mixture into an ice cream maker and churn according to the manufacturer's instructions until it reaches a soft-serve consistency.
