Prepare glass

Select a tall rocks glass (hurricane or Collins). Fill the glass halfway with crushed ice or a large handful of cubed ice to chill while you mix.

Combine ingredients

In a cocktail shaker or mixing tin large enough to hold the liquid and ice, add 2 oz dark rum, 1 oz light rum (if using), 4 oz pineapple juice, 1 oz orange juice, and 1 oz coconut cream.

Add ice and shake

Add a generous scoop of ice to the shaker (about 1 cup). Secure the lid and shake vigorously for 10–15 seconds until the outside of the shaker is cold and slightly frosted. This chills and slightly dilutes the drink and incorporates the coconut cream.

Strain and serve

Discard the ice from the chilled glass if you chilled it, then fill the glass with fresh crushed ice. Strain the shaken mixture into the prepared glass over the crushed ice. If you prefer, pour unstrained for a slightly thicker texture.

Garnish and finish

Grate a generous pinch (about 1/8–1/4 tsp) of fresh nutmeg over the top of the drink. Add a pineapple wedge and an orange slice to the rim, and drop in a cocktail cherry if desired. Serve with a straw and a small napkin.

Notes on balance

Taste before serving: adjust with a splash more pineapple or orange juice if too strong, or add a dash more coconut cream if you prefer creamier mouthfeel. The classic Painkiller is typically served with freshly grated nutmeg on top.
