Prepare and shake

Fill a shaker two-thirds with ice. Add 2 oz aged rum, 0.75 oz fresh lime juice, 0.5 oz fresh grapefruit juice, 0.5 oz orgeat, 0.25 oz demerara or simple syrup, and 2 dashes Angostura bitters. Cap the shaker and shake vigorously for 12–15 seconds to chill, dilute, and integrate the orgeat.

Glass and ice

Fill a double old-fashioned glass (or tiki mug) with about 1 cup of cracked pebble or crushed ice, packing it to form a small mound.

Strain and float

Double-strain the shaken mixture over the prepared crushed ice. Using the back of a bar spoon, gently float 0.5 oz dark rum on top by pouring it slowly so it layers.

Garnish & Finish

Grate about 1 tsp fresh nutmeg over the top of the drink. Express a grapefruit twist over the surface to spray oils, then drop the twist or place a thin grapefruit wheel on the ice. Optionally serve with a short straw and a garnish pick.
