Prepare ingredients
Measure and chill a coupe or rocks glass by filling it with ice water while you prepare the cocktail. If using fresh pineapple juice, strain to remove any pulp for a smoother texture. Have blueberry syrup ready (see note if making your own).

Dry shake
Add tequila, pineapple juice, blueberry syrup, fresh lime juice, heavy cream, egg white (if using), and simple syrup into a cocktail shaker without ice. Seal and dry-shake vigorously for 15–20 seconds. This emulsifies the cream and egg white to create a silky, foamy body.

Wet shake
Add a generous handful of ice to the shaker (about 6 medium cubes). Shake hard for 12–15 seconds until the shaker is very cold to the touch and the mixture is well chilled and slightly frothy.

Strain and serve
Discard ice water from the chilled serving glass. Double-strain the cocktail into the chilled glass through a fine mesh strainer (and the shaker strainer) to remove ice shards and any solids, producing a smooth, creamy texture.

Garnish and finish
Gently float 3–6 fresh blueberries on the foam and add a lime wheel or a small twist of lime peel on the rim. Optionally, lightly drizzle a few drops of blueberry syrup on top of the foam for color contrast. Serve immediately.

Notes — blueberry syrup (optional homemade)
To make about 6 oz blueberry syrup: combine 1 cup fresh or frozen blueberries, 1 cup sugar, and 1/2 cup water in a small saucepan. Bring to a gentle simmer and cook 8–10 minutes, mashing berries to release juices. Strain through a fine mesh sieve, pressing solids to extract liquid. Cool and store refrigerated up to 2 weeks. Use 3/4 oz per cocktail as called for above.
