Melt butter and soften garlic

Place a medium saucepan over medium-low heat. Add the unsalted butter and let it melt gently. Add the finely minced garlic and cook, stirring constantly, for 30–45 seconds until fragrant but not browned.

Sprinkle the all-purpose flour over the melted butter and garlic. Stir constantly with a whisk or wooden spoon for 1 minute to cook the raw flour flavor and form a smooth paste (roux). Keep the heat low so the roux does not brown.

Add milk and cream

Slowly pour in the whole milk while whisking to break up any lumps, then add the heavy cream. Increase heat to medium-low and continue whisking until the mixture is smooth and begins to steam. Do not boil — maintain a gentle simmer.

Thicken the sauce

Let the sauce simmer gently for 3–5 minutes, whisking occasionally, until it thickens enough to coat the back of a spoon. If it becomes too thick, add up to 2 tablespoons extra milk to reach desired consistency.

Incorporate cheeses and season

Lower the heat to low. Whisk in the softened cream cheese (if using) until fully melted and smooth. Gradually add the grated Parmesan cheese, about 1/4 cup at a time, whisking until melted before adding more. Stir in the salt, freshly ground black pepper, and nutmeg (if using). Taste and adjust seasoning.

Finish and garnish

Remove the saucepan from heat. Let the sauce sit for 1–2 minutes to thicken slightly. If necessary, thin with a splash of milk. Transfer to a small serving bowl or ramekin and sprinkle with chopped fresh parsley. Serve warm alongside pasta, steamed vegetables, or as a dipping sauce.
