Prepare glass
Chill a double old-fashioned glass or tiki mug by filling it with ice and cold water. Set aside while you build the cocktail.
Combine ingredients
In a cocktail shaker, add dark rum, pineapple juice, fresh lime juice, orgeat syrup, falernum, simple syrup and the angostura bitters. Add a large handful of ice (about 6-8 large ice cubes).
Shake
Secure the shaker and shake vigorously for 12–15 seconds until the shaker feels very cold and slightly frosty. This ensures proper dilution and chilling.
Strain
Double-strain the contents of the shaker over the cracked ice using a fine mesh strainer in addition to the shaker's strainer to remove small ice shards and pulp for a smooth texture.
Finish & garnish
Top with a light sprinkle of grated nutmeg (about 1/4 tsp) if using. Clap the mint sprig between your hands to release the oils, then tuck the mint and a pineapple wedge into the drink as a garnish. Optionally, add one more dash of angostura bitters on the top of the foam for aroma.
Serve
Serve immediately with a short straw. Sip and enjoy the balance of rich rum, bright pineapple and almond notes from the orgeat.
