Toast and prepare sandwich base

Split the English muffin or roll and toast cut sides until golden. Spread 1/2 tbs of the unsalted butter on the cut sides immediately after toasting so it melts in.

Cook sausage patties

Heat a nonstick or cast-iron skillet over medium heat. Add the two sausage patties (no additional oil needed if using typical breakfast sausage). Cook for about 4–5 minutes per side, pressing gently to flatten slightly, until internal temperature reaches 160°F (71°C) and exterior is browned. Transfer patties to a plate and keep warm. If patties are very large or thick, reduce heat after browning to finish through without burning.

Scramble eggs (folded round method)

Wipe the skillet if it has excess fat, leaving a thin film. Reduce heat to medium-low. Add remaining 1/2 tbs butter and let melt. In a small bowl, whisk the 2 large eggs with 1 tsp milk or cream, 1/4 tsp black pepper, and 1/2 tsp salt until blended but not foamy. Pour eggs into the skillet and let sit 10–15 seconds until edges set. Using a spatula, pull edges to the center, tilting the pan to let uncooked egg flow to the edges. Continue gentle folding for about 45–60 seconds until just-set but still moist — you want a slightly folded round that will sit on the sandwich. Alternatively, for a neat round: spray an 8–9 cm (3–3.5 in) metal ring or ramekin with nonstick spray, place in the skillet, pour eggs into the ring, and cook covered 1–2 minutes until set; remove ring.

Assemble and melt cheese

Place one cooked sausage patty on the bottom toasted muffin half. Top with the cooked egg. Place a slice of American cheese over the egg, and then the second sausage patty (or place cheese between patties if you prefer). Cover the skillet with a lid or place sandwich in a low oven (or microwave very briefly) for 30–45 seconds to fully melt the cheese and marry the layers. If you used butter on the top muffin half, place it on top at this time.

Finish and serve

Close the sandwich with the top toasted muffin half, press gently to compress slightly, slice in half if desired, and serve immediately. Adjust salt and pepper to taste.
