Measure and prep all ingredients: finely chop onion, bell pepper and celery; mince garlic; grate Parmesan; soften cream cheese if needed; slice/dice pepperoni if not pre-sliced.

Brown the proteins
Heat a large deep skillet or sauté pan over medium-high heat. Add the ground beef and break it up with a spatula. Cook 3–4 minutes until it starts to brown. Add the Italian sausage (remove casings and crumble) and continue to cook 4–5 minutes until both are well browned and no pink remains. Add the pepperoni and cook another 1–2 minutes so it releases some oils and toasts slightly. Using a slotted spoon, transfer most of the rendered fat out of the pan, leaving about 1 tablespoon for flavor; discard excess fat or reserve for another use.

Sauté vegetables
Reduce heat to medium. Add the butter to the same pan. When melted, add the chopped onion, bell pepper and celery (the classic Cajun ‘holy trinity’). Sauté 4–5 minutes until softened and translucent. Add the minced garlic and sauté 30–45 seconds until fragrant, stirring to avoid burning.
Build the Sauce

Make a roux and liquids
Sprinkle the all-purpose flour over the vegetables and stir constantly for 1–2 minutes to cook the raw flour taste, creating a light roux. Gradually whisk in the chicken broth until smooth, then whisk in the milk and heavy cream. Bring the mixture to a gentle simmer, stirring frequently to prevent scorching.

Add cheeses and seasonings
Lower heat to medium-low. Add the cream cheese in small pieces and whisk until fully incorporated and smooth. Stir in the grated Parmesan until melted. Add Cajun seasoning, smoked paprika, dried oregano, dried thyme, red pepper flakes (if using), salt and black pepper. Taste and adjust seasoning — add up to another 1/2 teaspoon salt or more Cajun seasoning for heat as desired.

Combine with meats
Return the browned meat and any accumulated juices to the pan, stirring to combine thoroughly. Simmer gently 3–5 minutes so flavors meld and sauce thickens to a creamy consistency that coats a spoon; if the sauce becomes too thick, thin with up to 2 tablespoons water or milk at a time.

Stir in chopped parsley and the optional lemon juice to brighten flavors. Adjust final seasoning to taste.

If serving with pasta: toss hot cooked pasta (about 12 oz drained) directly into the sauce in the pan until well coated; add a splash of reserved pasta water if needed to loosen. Serve immediately topped with extra Parmesan and parsley. If serving over rice or with bread, spoon sauce over portioned rice or serve alongside crusty bread.

Make-ahead/storage: cool to room temperature, then refrigerate in an airtight container up to 3 days or freeze up to 2 months. Reheat gently over low heat, adding a splash of milk to loosen.
