In a small bowl or heatproof cup, combine the unsweetened cocoa powder and granulated sugar. Add the hot water (about 2 tablespoons) and whisk vigorously until the mixture becomes a smooth, glossy paste with no lumps. Add the pinch of salt and whisk to combine. This concentrated paste ensures the cocoa fully dissolves and distributes evenly through the milk.

Combine with milk
Pour the 2 cups of cold (or lightly warmed if preferred) whole milk into a pitcher or two serving glasses. Stir in the vanilla extract. Add the cocoa paste to the milk and whisk or stir briskly until fully incorporated and the milk has a uniform chocolate color. If you prefer a richer chocolate milk, whisk in the optional 2 tablespoons heavy cream at this stage.

Notes on variations and storage
For a dairy-free version, substitute an equal amount of almond, soy, oat, or other plant-based milk. To make a larger batch, scale ingredients proportionally. Store leftover chocolate milk in a covered container in the refrigerator for up to 48 hours; shake or stir well before drinking because separation may occur.
