Heat milk and cream
In a medium saucepan over medium-low heat, combine 2 cups whole milk and 1/2 cup heavy cream. Warm gently until small bubbles form around the edges and steam rises, about 4–6 minutes. Do not let the mixture come to a full boil.
Mix cocoa and sugar
While the milk warms, in a small bowl whisk together 2 tablespoons unsweetened cocoa powder and 2 teaspoons granulated sugar with 1–2 tablespoons of the warm milk from the saucepan to form a smooth paste (this helps prevent lumps).
Combine chocolate
Stir the cocoa paste back into the saucepan. Add 2 ounces chopped bittersweet or semi-sweet chocolate and a pinch of salt. Keep the heat at medium-low and whisk constantly until the chocolate is fully melted and the mixture is smooth and glossy, about 2–3 minutes.
Notes: For a richer drink, use all cream or add an extra ounce of chocolate. For a lighter version, substitute low-fat milk for whole milk. To make vegan, use a plant-based milk and omit heavy cream or substitute with coconut cream.
