Mix meatball ingredients
In a large bowl, combine ground beef (or beef/pork), breadcrumbs, milk, egg, grated Parmesan, minced garlic, chopped parsley, dried oregano, salt, and black pepper. Use your hands or a spoon to mix until just combined — do not overwork the mixture to keep meatballs tender.
Form meatballs
Portion the mixture into evenly sized meatballs about 1 1/4 inches (roughly 1.5 tablespoons or golf-ball size). You should get about 16–18 meatballs. Roll each gently between your palms to form smooth balls; avoid packing them tightly.
Brown meatballs
Heat 2 tablespoons olive oil in a large skillet over medium-high heat. When the oil shimmers, add meatballs in a single layer without crowding (work in batches if necessary). Brown on all sides, turning carefully, about 6–8 minutes total — they do not need to be fully cooked through. Transfer browned meatballs to a plate and set aside.
Tomato Sauce
Sauté aromatics
In the same skillet (wipe out excessive fat leaving a little for flavor) or in a separate large saucepan, add 1 tablespoon olive oil and set over medium heat. Add the finely chopped onion and sauté until translucent, about 5–6 minutes. Add the minced garlic and cook 30–60 seconds until fragrant, stirring so it doesn't burn.
Build the sauce
Stir in the tomato paste and cook 1 minute to deepen flavor. Add the crushed tomatoes, dried basil, dried oregano, sugar (if using), red pepper flakes (if using), salt, and black pepper. Stir to combine and bring to a gentle simmer.
Simmer with meatballs
Return the browned meatballs to the sauce, nestling them into the tomatoes. Reduce heat to low and simmer, partially covered, for 18–22 minutes until meatballs are cooked through (internal temperature 160°F/71°C) and sauce has slightly thickened. Stir occasionally and spoon sauce over meatballs to keep them moist.
Pasta & Finishing
Combine pasta and sauce
Add the drained linguine to the skillet or saucepan with the sauce and meatballs. Toss gently to combine, adding reserved pasta water a few tablespoons at a time as needed to loosen and coat the pasta (use up to 1/2 cup). Cook together for 1–2 minutes so the pasta absorbs some sauce.
Finish and serve
Taste and adjust seasoning with salt and pepper. Sprinkle torn fresh basil and grated Parmesan over the pasta. Serve hot, dividing into 4 portions and top each with additional Parmesan if desired.
