Prepare glass
Chill a rocks glass or short tumbler in the freezer for 5–10 minutes, or fill with ice water while you prepare the drink. Empty the glass just before pouring.
Build and shake
Into a cocktail shaker, add 4 ice cubes, 2 oz vodka, 1 oz coffee liqueur, 1 oz caramel syrup, and 2 oz whole milk. Secure the lid and shake vigorously for 10–15 seconds until the outside of the shaker is cold and the mixture is well emulsified.
Strain
Fill the chilled glass with fresh ice (about 3–4 cubes). Use a Hawthorne or fine strainer to pour the shaken mixture over the ice into the glass, leaving any excess foam in the shaker if desired.
Garnish & Finish
Top the drink with about 2 tablespoons of whipped cream, either from a can or lightly whipped fresh cream. Hold back slightly to create a neat dome over the drink.
Drizzle 1 tablespoon caramel sauce in a spiral or zigzag pattern over the whipped cream. If using, float 2 coffee beans on top for a decorative accent.
Serve immediately with a short spoon or straw. Optionally provide a napkin or coaster as the caramel can drip.
