Prepare aromatics
Heat a large heavy-bottomed pot over medium heat. Add the neutral oil (2 tbs). When shimmering, add the diced onion and a pinch of salt. Sauté for 4–5 minutes until translucent. Add the minced garlic and minced ginger and sauté 1–2 minutes until fragrant, taking care not to burn the garlic.
Incorporate coconut milk
Stir in the full-fat coconut milk (400 ml) and, if using, the ginger juice (1 tsp). Rewarm gently over low heat; do not boil vigorously once coconut milk is added to avoid separation.

Season and balance
Taste the soup and add the fish sauce or soy sauce (1 tbs) for umami, brown sugar (1 tsp) to balance acidity, lime juice (2 tbs) for brightness, sea salt (1.5 tsp overall, adjusting to taste), and freshly ground black pepper (1/2 tsp). Stir and simmer gently 2–3 minutes to allow the flavors to meld. Adjust lime, salt, or sugar as needed.
Optional flavor boost
If you want extra lemon grass essence, stir in 1 tsp of lemongrass oil (or a few drops) at the end for concentrated aroma. If you prefer more ginger heat, add the optional 1 tsp ginger juice now, tasting as you go.

Ladle the soup into warm bowls. Swirl a little reserved coconut milk (50 ml) on top for a creamy finish and visual contrast.
Notes on texture and storage
If the soup is too thick, thin with up to 100 ml additional stock or water. Refrigerate leftovers in an airtight container for up to 3 days; reheat gently. The soup can be frozen for up to 2 months—thaw and reheat slowly, stirring in a splash of coconut milk or stock if it has separated.
