Method
Prepare ramekins and dulce de leche
Prep ramekins
Preheat oven to 425°F (220°C). Generously butter four 6-ounce (180 ml) ramekins using about 1/4 teaspoon to 1/2 teaspoon butter each (reserve remaining butter listed in ingredients if needed). Dust the inside of each ramekin with cocoa powder, tapping out any excess so a thin even coating remains.
Portion dulce de leche
Spoon 1 1/2 tablespoons (approximately 22–24 g) of thick dulce de leche into four small balls using a melon baller or two spoons and place each ball on a sheet of parchment or a lightly buttered plate. Chill in the freezer 10–15 minutes to firm slightly — this helps the center stay molten while the cake bakes.
Make cake batter
In a heatproof bowl set over (not touching) a small pot of simmering water (double boiler), combine the chopped semi-sweet chocolate and 6 tablespoons unsalted butter. Stir occasionally until fully melted and smooth, about 3–5 minutes. Remove from heat and let cool 2–3 minutes.
Whisk the granulated sugar into the slightly cooled chocolate mixture until incorporated. Add the whole eggs and egg yolks one at a time, whisking briskly after each addition until the mixture is glossy and slightly thickened.
Stir in the vanilla extract and a pinch of salt. Sprinkle the tablespoon of all-purpose flour over the batter and fold gently with a spatula until just combined and no streaks remain. Do not overmix — the batter should be thick and smooth.
Assemble and bake
Divide about 3 tablespoons of batter evenly among the prepared ramekins (fill each about halfway). Place a frozen dulce de leche ball in the center of each ramekin, pressing down slightly so it's covered but still leaving space for more batter.
Top each dulce de leche center with the remaining batter, dividing it evenly so each ramekin is about 3/4 full and the dulce de leche is fully enclosed.
Place ramekins on a baking sheet and bake in the preheated 425°F (220°C) oven for 11–13 minutes. Bake until the edges are set and the tops look puffed but the center still trembles when gently shaken. Exact time depends on oven and ramekin size — check at 10 minutes if unsure.
Finish and serve
Dust the tops lightly with powdered sugar if using. Serve immediately with a scoop of vanilla ice cream or a dollop of whipped cream to complement the hot molten dulce de leche. To enjoy the lava, break the top with a spoon to reveal the warm flowing center.
